When are the NCCO and audio file for a stream downloaded?

Answer
When using the Voice API, Vonage will download the file when it is required.
NCCO
The Nexmo Call Control Object (NCCO) is downloaded when a call is answered. For an outbound call, this will be when the first recipient answers. For an inbound call, Vonage will answer the call and then download the NCCO.
Stream Audio File
The audio file for a stream action is downloaded when it is needed. This means that the MP3 or WAV file will be downloaded when the Voice API reaches the stream action. If you have actions before your stream, these will be processed before the file is downloaded.
