Symptom

Vonage SMPP API rejects submit_sm and returns a submit_sm_resp with command_status = 0x00000067 (hex) OR 103 (decimal).

    3.4 spec says 0x67 is ESME_RQUERYFAIL "query_sm request failed" - is this correct?
